Iron is another metal postulated for its relationship to alopecia, though the connection between iron deficiency and hair loss is still in debate (24,25). This link has been supported in the literature (26), and some centers, such as Cleveland Xxxxxx Foundation, have taken the position that hair loss is improved when iron defi- ciency is treated, regardless of the absence or presence of anemia (27). Other studies, however, did not find this association in their investigations (28,29). We believe more definitive evidence is needed to confirm the connection between iron deficiency and hair loss. The current literature considers the adequate ferritin level to be 40mg/L, though studies have supported a range of 10–70mg/L (29,30)

The association of low serum ferritin level and hair loss has been debated over the years. There are ongoing controversies for low serum ferritin levels to be designated as nutritional deficiency triggering hair loss [18]. In order to reverse severe hair loss some authors suggest serum ferritin level should be maintained above 40ng/dl [17] or 70ng/dl [19]. Adequate dietary intake and oral administration of ferrous sulfate 300mg three to four times a day is an effective, initial therapy [20]. Iron supplementation is given for 3-6 months till stores are replenished [20,21].

The evidence for this can be found in Table 2, which shows the most frequent abnormal nutrient in women presenting with CTE is a low serum ferritin. Sixty-five per cent had a concentration below 40 dg / dL the lowest control value obtained in women without hair loss37 a level just above the lower limit of normal for males quoted in some laboratories.34 However 96% had a serum ferritin below 70 ng / dL, the upper 99% confidence limit for iron staining in the bone mar- row.45 Low serum ferritin concentrations in women with hair loss have recently been reported by Kantor et al.47 supporting the initial observation of Rushton in 1993.39 {...} What level of serum ferritin to use is still not fully resolved but 70 ng / dL, with a normal erthrocyte sedimentation rate (< 10 mm / h) is a recommended level to achieve. The level of supplementation or dietary change needed to maintain serum ferritin concentrations above 70 ng / dL is the subject of continuing research.

It is our practice at the Cleveland Xxxxxx Foundation to screen male and female patients presenting with hair loss, both cicatricial and noncicatricial, for iron deficiency by obtaining a complete blood cell count, red blood cell indices, and serum ferritin concentration. We treat iron deficiency, with or without anemia, through dietary modification and, when necessary, oral iron supplementation. Although this practice is not evidence based per se, in our experience we believe that treatment for hair loss is enhanced when patients maintain a serum ferritin concentration greater than 70 ng/mL.

Low serum ferritin, Vitamin D, and zinc levels have been shown to be the possible contributory factors in hair loss.[12] Iron deficiency has been demonstrated to be more common in patients with hair loss conditions such as FPHL, TE, and alopecia areata;[12],[13],[14],[15],[16] however, other studies found no significant association between iron deficiency and hair loss.[17],[18] Mean serum ferritin level (76.2 µg/L) in our patient group was much higher compared to the previous studies (37.3 µg/L, 23.9 µg/L).[12],[13] This difference could be explained by diet patterns, genetic and ethnic variations compared to other study populations. While the ?association and definition of iron deficiency in hair loss remain to be clarified, we routinely prescribe iron supplement to patients with a serum ferritin level below 50 µg/L.
